Department of Veterans Affairs (VA) Finger Lakes Healthcare System (FLHCS)

Period of Performance: March 15, 2025, through March 14, 2026, with three one (1) year option periods.

Place of Performance: Bath VA Medical Center at 76 Veterans Avenue Bath, NY 14810

Schedule: Monday through Friday, 8:00 a.m. – 4:30 p.m.

Contractor will provide MOD/Hospitalist Physician Services during non-administrative which includes all weekends, holidays, evenings, and nights (WHEN). Work Schedule and Scheduling: The contractor will be responsible for scheduling and providing Hospitalist coverage for the following shifts: Monday through Friday: 4:00 p.m. - 8:00 a.m. Weekends: 8:00 a.m. Saturday through 8:00 a.m. Monday Federal Holidays: 8:00 a.m. on the holiday through 8:00 a.m. the day after the holiday.

Position Requirements:

  • License: The Contractor’s physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor’s physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application will not be considered for the purposes of this contract. A New York State license is preferred, but not required.

  • Board Certification/Eligibility: All contract physicians shall be board certified and/or eligible in Internal Medicine or Family Medicine

  • Be currently certified by the American Heart Association in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).ACLS is not a substitute for BLS certification. All continuing key courses required for maintaining certification must always be kept up to date. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance.

  • Active DEA registration

  • Must be a U.S. citizen

Duties and Responsibilities may include but are not limited to the following:

Direct Patient Care: 90% of the time involved in direct patient care.

  • The Contractor shall provide the following staff and is responsible for providing enough qualified contract physicians to maintain coverage without gaps.

  • The contractor is responsible for providing MOD/Hospitalist 24-hour coverage on weekends and any Federal Holiday, along with coverage for WHEN hours 1600 to 0800.All contract providers shall be board-certified and/or eligible in Internal Medicine or Family Medicine.

Contract providers, as appropriate and within the scope of practice/privileging, shall be responsible for providing general medicine care to hospitalized patients, including, but not limited to:

  • Contract physician acts as Medical Officer of the Day (MOD) during non-administrative hours.

  • MOD/Hospitalist shall be responsible for admitting and discharging patients during shifts. This includes, but is not limited to, community living centers (CLC), and domiciliary (Mental Health Residential Rehabilitation and Treatment Program).The Medical Center has a total of 169 bed for all Finger Lakes CLC’s and 170 bed Domiciliary (DOM).

  • MOD/Hospitalist shall be responsible for rounding on CLC and DOM floors at the beginning, ending, and periodically throughout the assigned shift.

  • The MOD/Hospitalist shall help as needed with the completion of the CLC Provider Periodic Reassessments. These are scheduled to be completed for all new admissions at 30, 60 and 90 days, and long-term residents every 60 days.

  • The MOD/Hospitalist will be responsible for performing an evaluation to determine suitability for direct admission to domiciliary and write admitting orders for pre-approved DOM admissions who report to the facility when DOM Attending is not available and UCC is closed.

  • The MOD/Hospitalist shall be familiar with Code Blue Policy. The contractor will serve as code director while on duty. The contract physician shall be familiar with Code Blue Policy.

  • MOD/Hospitalist shall participate in calls with VA or private physicians regarding admissions, transfers or referrals.

  • The On-call Psychiatrist is to be contacted by contract physicians regarding any suicidal/homicidal or acutely psychotic patients to determine appropriate management.

  • MOD/Hospitalist will be familiar with procedures regarding determination of blood compatibility for transfusions and be responsible for verification of compatibility.

  • MOD/Hospitalist is also responsible for providing care to Medical Center personnel when Employee Health/UCC is closed.

  • In the event of a disaster, the MOD/Hospitalist will be responsible for triage and treatment of casualties until the arrival of additional VA medical staff, and it is deemed that the contract MOD is no longer needed.

  • MOD/Hospitalist shall be responsible for providing medical care as needed during shift.This includes community living center (CLC), and DOM.

  • MOD/Hospitalist shall provide evaluation and management of clinical issues and changes arising during the coverage period.

  • MOD/Hospitalist/UCC providers will respond to calls from DOM and CLCs at both Bath and Canandaigua for medical concerns in accordance with VA Policy. Modality of care will depend on the needs of the patient and may include phone, VVC, CVT, or face-to-face.

ADMINISTRATIVE: 10% of the time, not involved in direct patient care

  • Quality Assurance/Quality Improvement (QA/QI) Documentation:Contract physicians shall participate in continuous quality improvement activities occasionally as required by the VAMC Chief of Service, Chief of Staff, or designee.

  • QA/QI Documentation: The contract physicians shall complete the appropriate QM documentation pertaining to all procedures, complications, and outcomes of examinations.

  • Specialized CPRS Documentation Procedures:Patient documentation is completed using the Computerized Patient Record System (CPRS). All contract physicians shall participate in a training session prior to providing any patient care services at the VA.Contract physicians shall document care provided and select appropriate Evaluation and Management (E&M) procedure codes in CPRS. Documentation must be sufficient to support both the E&M and procedure codes. Documentation and coding functions must be completed by the end of each patient care encounter.

  • Patient Safety Compliance and Reporting: Contract physicians shall follow all established patient safety and infection control standards of care. Contract physicians shall make every effort to prevent medication errors, falls, and patient injury caused by acts of commission or omission in the delivery of care. All events related to patient injury, medication errors, and other breaches of patient safety shall be documented in the medical records of those impacted and disclosed to the patient and surrogate. As soon as practicable (but within 24 hours) contract physician shall notify the COR of the incident and submit an entry in the VA Patient Safety Reporting System, following up with the COR as required or requested.

  • Customer Service: Contract physicians shall refer all patient/customer service issues to the Chief of Medicine, Nurse Manager, COR, or designee.
